Zusammenfassung

Total Life in the United States is seeking nurse practitioners to lead longevity days at partner senior living communities in the Redding area. This contract, on-site role starts with education talks and moves to one-on-one longevity visits with residents, under the guidance of our Chief Medical Officer.

You will travel to assigned communities, perform comprehensive geriatric assessments, review medications, and coordinate care with primary providers.

Qualifikationen

• Active California RN license and NP certification required.
• Active NP furnishing number in California.
• Experience with Medicare documentation and E/M coding.

Aufgaben

• Conduct 45–60 minute longevity visits based on geriatric assessment.
• Evaluate weight management eligibility and GLP-1 use where appropriate.
• Coordinate follow-up with primary care providers and specialists.
• Document encounters per Medicare standards in the EHR.
• Represent Total Life professionally with residents and families.
• Travel to assigned senior living communities in the Redding area.
