Total Life, Inc. is seeking nurse practitioners for an in-person contract role in the Capital Region (Albany-Schenectady-Troy). You will lead longevity days at partner senior living communities, starting with a short educational talk and then one-on-one visits.

You'll be trained by our CMO, travel to assigned communities, and work a weekday schedule with hours that grow as bookings increase. This role focuses on geriatric assessment, medication review and coordination with primary care.

Qualificações

• Active NY RN license and NP certification required.
• Current NY NP board certification.
• DEA registration and NY EPCS setup.
• 3 hours yearly prescriber education.
• Experience with Medicare documentation and E/M coding.
• Clinical experience caring for adults 65+.

Responsabilidades

• Conduct longevity visits 45–60 minutes after initial education talk.
• Perform comprehensive geriatric assessment and medication review.
• Identify residents needing follow-up care and coordinate with PCP.
• Document encounters to Medicare standards in the EHR.
• Represent Total Life professionally with residents and families.
• Travel to assigned senior living communities in the Capital Region.

Conhecimentos

Active NY RN license

DEA registration

Prescriber education (3 hours/3 years)

Medicare documentation experience
