Job summary

Total Life is seeking licensed nurse practitioners for an in-person contract role in the Connecticut shoreline area (Groton-New Haven). You will lead longevity days at partner senior living communities, delivering education and one-on-one visits to residents, with support from our CMO and team.

The position emphasizes geriatric assessment, medication review, cognitive screening and collaboration with primary care providers. Weekday site days grow with bookings.

Qualifications

• Active, unrestricted Connecticut APRN license.
• Current national NP board certification.
• Active DEA registration and Connecticut Controlled Substance Practitioner Registration.
• Independent practice eligibility in CT or written collaborative agreement.
• Clinical experience caring for adults 65+.
• Experience with Medicare documentation and E/M coding.
• Malpractice insurance covering in-person care.
• Valid driver's license and reliable transportation.
• Onboarding requirements including background check and TB screening.

Responsibilities

• Conduct 45–60 minute longevity visits based on a geriatric assessment.
• Evaluate weight management options, including GLP-1 when appropriate.
• Coordinate follow-up care with primary care providers and specialists.
• Document encounters to Medicare standards in the EHR.
• Represent Total Life professionally with residents and staff.
• Travel to assigned senior living communities in CT shoreline area.
