Job summary

Total Life Inc. is seeking nurse practitioners for an in-person contract role in the Capital Region (Albany–Schenectady–Troy) to lead longevity days at partner senior living communities.

Each day opens with an educational talk for residents, followed by one-on-one longevity visits. You will be trained by our Chief Medical Officer, perform comprehensive geriatric assessments, document for Medicare, and coordinate with residents’ primary care providers.

Qualifications

• Group presentation and public speaking to residents.
• Comprehensive geriatric assessment experience.
• Experience with Medicare documentation and E/M coding.

Responsibilities

• Conduct 45–60 minute longevity visits after geriatric assessment.
• Evaluate weight-management eligibility and GLP-1 therapy when appropriate.
• Coordinate follow-up care with primary care providers.
• Document encounters to Medicare standards in our EHR.
• Represent Total Life professionally with residents and staff.
• Travel to Capital Region senior living communities.
