Job summary

Total Life, Inc. is seeking Nurse Practitioners for an in-person contract role in the Grand Junction area to lead longevity days at partner senior living communities. You will present to residents, then conduct one-on-one longevity visits, supported by the CMO and team.

You will be trained in geriatric assessment, cognitive tools, and Medicare documentation, with a flexible contractor schedule and mileage reimbursement for site days.

Qualifications

• Active Colorado APRN Registry listing and Board certification.
• Colorado RN license (or multistate) with APRN privileges.
• Full prescriptive authority in Colorado or provisional with mentorship.
• DEA registration and ability to prescribe controlled substances.
• Registration with Colorado Prescription Drug Monitoring Program (PDMP).
• Experience with Medicare documentation and E/M coding.
• AGNP/geriatric-focused NP certification or Family NP with geriatrics focus.

Responsibilities

• Conduct 45–60 minute longevity visits based on comprehensive geriatric assessment.
• Evaluate residents for weight management with GLP-1 therapy when appropriate.
• Identify residents needing follow-up care and coordinate with primary care providers.
• Document encounters to Medicare standards in the EHR.
• Represent Total Life professionally with residents, families, and staff.
• Travel to Grand Junction area senior living communities for site days.
