Total Life, Inc. is seeking Connecticut-based nurse practitioners to lead longevity days at partner senior living communities. This in-person contract role focuses on education and one-on-one longevity visits, with training from our CMO and ongoing support.

Day to day includes geriatric assessment, medication review, and coordination with primary care providers. Site days are on weekdays in the Connecticut shoreline area, about 4–8 hours per day, with flexible hours and travel between

Qualifikationen

• Active, unrestricted CT APRN license and CT RN license.
• Current national NP board certification.
• Experience with Medicare documentation and E/M coding.
• Comfort presenting to groups and older adults/families.
• Willingness to travel to CT shoreline communities.
