Job summary

Total Life seeks nurse practitioners for an in-person contract role in the Chicago suburbs (Joliet-Palatine) to lead longevity days at partner senior living communities. Each day includes an educational talk for residents, followed by one-on-one longevity visits.

You'll be trained by our Chief Medical Officer and work with a team to complement residents’ ongoing care. Your site days are on weekdays with 4–8 hours per day; hours may grow as bookings increase.

Qualifications

• Active Illinois APRN license required.
• Current national NP board certification.
• DEA registration and Illinois mid-level practitioner license required.
• Ability to practice under a written collaborative agreement unless Full Practice Authority.
• Experience with Medicare documentation and E/M coding.
• Evidence of malpractice insurance or ability to obtain before first site day.

Responsibilities

• Conduct 45–60 minute longevity visits based on a comprehensive geriatric assessment.
• Evaluate eligibility for weight management including GLP-1 therapy.
• Coordinate follow-up care with residents’ primary care providers.
• Document encounters in Medicare-compliant EHR.
• Represent Total Life professionally with residents and families.
• Travel to assigned senior living communities in the Chicago suburbs (Joliet-Palatine) area.
