Job summary

Total Life Inc. is seeking nurse practitioners for an in-person contract role in the Merced–Fresno area to lead longevity visits at partner senior living communities. You will start with educational talks and progress to one-on-one longevity visits, supported by our CMO and team.

Responsibilities include comprehensive geriatric assessments, medication review, and coordinating follow-up care with residents' primary clinicians. On-site travel is required weekly.

Qualifications

• Active, unrestricted California RN license and NP certification.
• NP Furnishing Number from the California BRN.
• Current national NP board certification.
• Active DEA registration in California with CURES.
• Ability to practice under standardized procedures with physician supervision on site.

Responsibilities

• Conduct 45–60 minute longevity visits based on geriatric assessment.
• Evaluate residents for weight management when appropriate.
• Identify and coordinate follow-up care with primary care providers.
• Document encounters accurately per Medicare standards in the EHR.
• Represent Total Life professionally with residents, families and staff.
• Travel to Merced-Fresno area senior living communities.
