Job summary

GuideStar Eldercare is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to deliver neurobehavioral health services in long-term care facilities. You will collaborate with a multidisciplinary team to provide holistic care for older adults with neurocognitive and psychiatric conditions.

You will evaluate residents, offer therapy, and coordinate care with facilities, PCPs, and families as needed.

Qualifications

• Must collaborate with an interdisciplinary treatment team.
• Strong time management and organizational skills.
• Able to function autonomously with regional Area Manager support.
• Master’s degree in Mental Health Counseling or Social Work.
• Currently licensed as an LMHC or LCSW in the applying state.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy or insight-based therapy when indicated.
• Coordinate the patient’s care plan with the facility, PCP, colleagues, and engaging with the family.
