Job summary

GuideStar Eldercare is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to provide neurobehavioral health services in long-term care facilities.

You will partner with a multi-disciplinary team to support residents with neurocognitive disorders and other neurologic or psychiatric conditions, conducting evaluations and care planning with families and facility staff.

Qualifications

• Masters degree in Mental Health Counseling or Social Work.
• Currently licensed as LMHC or LCSW in the state you are applying.
• Ability to work as part of an interdisciplinary treatment team.
• Good time management skills.
• Able to function autonomously with regional Area Manager support.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy, or when indicated, insight-based therapy.
• Coordinate the patients care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
