Job summary

GuideStar Eldercare seeks a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to deliver neurobehavioral health services in long-term care facilities. You will collaborate with a multidisciplinary team to support elderly residents with neurocognitive and psychiatric conditions, focusing on holistic, continuous care.

Qualifications

• Must be able to work as part of an interdisciplinary treatment team.
• Strong time management skills required.
• Able to function autonomously with regional Area Manager support.
• Master’s degree required in Mental Health Counseling or Social Work.
• Current LMHC or LCSW licensure in state of application.

Responsibilities

• Provide evaluation.
• Deliver condition-specific support or insight-based therapy as indicated.
• Coordinate the patient’s care plan with facility, PCP, colleagues, and engage with family as needed.
