Job summary

GuideStar Eldercare seeks an LCSW or LPC to deliver neurobehavioral health services in long-term care facilities, collaborating with a multidisciplinary team to support elderly residents with neurocognitive disorders and related conditions.

You will perform evaluations and provide condition-specific therapy, coordinate care with facilities and PCPs, and engage families to ensure continuity of care and well-coordinated treatment. EMR documentation is required.

Qualifications

• Master’s degree in Mental Health Counseling or Social Work required.
• Licensed LMHC or LCSW in the applying state.
• Ability to work as part of an interdisciplinary treatment team.
• Good time management skills.
• Able to function autonomously with regional Area Manager support.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy or insight-based therapy as indicated.
• Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
