Job summary

GuideStar Eldercare seeks a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to deliver neurobehavioral health services to residents in long-term care facilities. You will work with a multidisciplinary team to address neurocognitive disorders and other neurologic or psychiatric conditions.

The role includes evaluating residents, providing therapy, coordinating care with facilities and PCPs, and communicating with families as needed.

Qualifications

• Ability to work as part of an interdisciplinary treatment team.
• Good time management skills.
• Able to function autonomously with regional Area Manager support.
• Master's degree in Mental Health Counseling or in Social Work.
• Currently licensed as an LMHC or LCSW in the state in which you are applying.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy, or when indicated, insight-based therapy.
• Coordinate the patient's care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
