Job summary

GuideStar Eldercare seeks a licensed clinician to deliver neurobehavioral health services to residents in long-term care facilities in Texas. You will collaborate with a multidisciplinary team to provide holistic care for elderly patients with neurocognitive and psychiatric conditions.

You will evaluate patients, provide therapy, and coordinate care plans with facilities and PCPs while engaging families as needed.

Qualifications

• Must be licensed LMHC or LCSW in Texas.
• Strong teamwork within an interdisciplinary treatment team.
• Ability to manage time effectively and work autonomously with regional support.

Responsibilities

• Provide evaluation and assessment for residents.
• Deliver condition-specific support or insight-based therapy as indicated.
• Coordinate care plans with facilities, PCPs and colleagues; engage family when appropriate.
