Job summary

GuideStar Eldercare is seeking a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) to provide neurobehavioral health services to residents in long-term care facilities. You will collaborate with a multidisciplinary team to deliver holistic, continuity of care for elderly individuals with neurocognitive disorders and other neurologic or psychiatric conditions.

• Part of an interdisciplinary treatment team
• Good time management skills
• Autonomous work with regional Area Manager support
• Must hold a Master’s degree in Mental Health Counseling or Social Work
• Current LMHC or LISW license in MN

Responsibilities

• Provide neurobehavioral health services to residents in long-term care facilities
• Partner with specialty clinicians to deliver holistic, continuous care
• Collaborate within an interdisciplinary treatment team to optimize patient outcomes
