Job summary

GuideStar Eldercare seeks a part-time Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Social Worker (LISW) to deliver neurobehavioral health services in long-term care facilities. You will work with a multi-disciplinary team to support elderly residents with neurocognitive disorders and related psychiatric conditions.

Qualifications

• Must work as part of an interdisciplinary treatment team.
• Good time management skills.
• Able to function autonomously with regional Area Manager support.
• Master’s degree in Counseling or Social Work.
• Currently licensed as LPCC or LISW in the state of application.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy or insight-based therapy when indicated.
• Coordinate the patient’s care plan with the facility, PCP, GuideStar colleagues, and engage with the family as indicated.
