Job summary

GuideStar Eldercare is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to deliver neurobehavioral health services in long-term care facilities. You will collaborate with a multidisciplinary team to support elderly residents with neurocognitive disorders and related conditions.

You will evaluate residents, provide therapy when indicated, and coordinate care plans with facilities, PCPs, and families, while documenting in an EMR system.

Qualifications

• Master’s degree in Mental Health Counseling or Social Work.
• Current LMHC or LCSW license in the applying state.
• Ability to work as part of an interdisciplinary treatment team.
• Strong time management and autonomous work capability.
• Experience with geriatric neurocognitive and psychiatric conditions is a plus.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy, or when indicated, insight-based therapy.
• Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
