Job summary

GuideStar Eldercare seeks a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to deliver neurobehavioral health services in long-term care facilities. You will collaborate with a multidisciplinary team to support elderly residents with neurocognitive disorders and related neurological and psychiatric conditions.

The role includes evaluation, condition-specific therapy, and coordinating care plans with facilities, PCPs, colleagues, and families, while documenting

Qualifications

• Master’s degree in Mental Health Counseling or in Social Work.
• Current LMHC or LCSW licensure in the applying state.
• Ability to work on an interdisciplinary treatment team.
• Strong time management and self-direction.

Responsibilities

• Provide evaluation.
• Provide condition-specific support therapy, or when indicated, insight-based therapy.
• Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
