Job summary

Longevity Health seeks an Advanced Practice Practitioner to float across markets, providing collaborative primary care to nursing home residents and Longevity Health Plan members. The role covers multiple settings and requires flexible scheduling and travel.

As an NP/PA, you will support Model of Care activities, lead advance care planning, and collaborate with care teams to close quality gaps while maintaining patient-centered care in the aging population.

Qualifications

• Minimum 6 months experience as NP/PA; geriatric/hospice or palliative care preferred.
• Willingness to travel statewide as directed.
• Active state NP/PA license and DEA license required.
• Strong communication with physicians, patients, families.

Responsibilities

• Provide collaborative primary care to Longevity Health Plan members in various settings.
• Execute Model of Care activities and develop advance care planning.
• Coordinate with care teams on authorizations, services, and quality gaps.
• Communicate with PCPs and family members to align goals of care.
• Travel within market as required and manage caseloads.
