Longevity Health Plan is seeking an Advanced Practice Practitioner (APP) – Market Floater to medically manage a panel of nursing home level of care members, collaborating with primary care teams and delivering the Longevity Health Plan Model of Care.

Qualifications

• Active NP or PA licensure in the applicable state.
• Board certification in nursing or physician assistant with geriatric experience preferred.
• DEA license required.
