Job summary

Longevity Health is seeking an Advanced Practice Practitioner (APP) – Market Floater, a licensed and board-certified NP or PA, to support Longevity Health Plan members across Texas. The APP will manage a panel of nursing home residents, perform comprehensive assessments, and develop individualized plans of care with the primary care team.

Expect travel 3–5 days weekly, with all travel expenses covered; the role emphasizes treating in place, coordination with care teams, and strong communication

Qualifications

• Licensed and board-certified NP or PA.
• Market Floater role: travels across Texas; 3-5 days weekly; expenses covered.

Responsibilities

• Provides collaborative primary care and services to Longevity Health Plan members across nursing facilities.
• Executes Longevity Health Plan Model of Care activities and care planning.
• Delivers comprehensive assessments and develops individualized plans of care.
• Attends nursing facility care team meetings and communicates with families.
• Documents health status, functional status, and diagnoses accurately.
• Supports market goals and coordinates with PCPs and care teams.
