Longevity Health Plan seeks an Advanced Practice Practitioner (APP) Market Floater to medically manage a panel of nursing home patients across multiple markets.

The APP will perform comprehensive assessments, develop and implement individualized care plans, and participate in nursing facility care team meetings while coordinating with residents' families and primary care physicians.

Travel across state markets is required; supervision varies by market and adherence to Model of Care is expected.

Qualifikationen

• Must be a licensed NP or PA with board certification.
• Willingness to travel statewide as directed.
• Experience in nursing home or geriatric settings preferred.
• Strong clinical skills in assessment, diagnosis, and treatment.
• Excellent communication with physicians, patients and families.

Aufgaben

• Provide collaborative primary care to Longevity Health Plan members.
• Deliver Model of Care activities and care planning.
• Coordinate with care teams and family members; attend care meetings.
• Handle prior authorization and quality initiatives; support STARs/HEDIS.
• Communicate with PCPs and care teams about goals and changes.
