Job summary

Longevity Health Plan is seeking an Advanced Practice Practitioner to serve as a Market Floater across nursing facility settings. The APP will manage a panel of Longevity Health Plan enrolled members, perform comprehensive assessments, and develop individualized plans of care in partnership with the primary care team.

The role requires board-certified NP/PA licensure, unrestricted state license, DEA registration, and the ability to travel statewide.

Qualifications

• Minimum 6 months experience as NP/PA with geriatric, hospice, or palliative care preferred.
• Experience in nursing home environment strongly desired.
• Ability to travel statewide as directed and agreed.
• DEA license required and active NPI/Medicare provider numbers.

Responsibilities

• Provide collaborative primary care to Longevity Health Plan members in nursing facility settings.
• Deliver Model of Care activities and advanced care planning.
• Coordinate with care teams, PCPs, and families to align goals of care.
• Communicate member changes to care teams and telehealth partners.
• Balance caseloads with team support needs and promote member engagement.
