Job summary

Longevity Health is seeking an Advanced Practice Practitioner (APP) – Market Floater to manage a panel of nursing home patients across Texas, floating to cover vacancies as needed. A licensed NP or PA with board certification is required to participate in the Longevity Health Plan Model of Care.

The APP will perform comprehensive assessments, develop individualized care plans, attend facility meetings, and communicate with families and the care team to achieve patient-centered outcomes.

• Licensed and board-certified NP or PA.

Responsibilities

• Effectively medically manage a panel of Longevity Health Plan enrolled nursing home level of care/institutional members in partnership with primary care team.
• Develop and deliver the member's individualized plan of care, attend nursing facility care team meetings, and communicate with family/loved ones.
• Deliver Longevity Health Plan Model of Care activities, execute treating in place, and manage care planning.
• Coordinate with PCPs, care teams, and telehealth partners for changes in member condition.
• Assist with prior authorization, clinical operations, and quality improvement activities as needed.
