Job summary
Longevity Health is seeking an Advanced Practice Practitioner (APP) – Market Floater to support Longevity Health Plan members across markets. The role requires a licensed and board-certified NP or PA with experience in geriatric care, and involves traveling within the state market to fill vacancies and cover absences.
You will manage a panel of nursing home in-scope members, develop individualized care plans, participate in facility care team meetings, and communicate with families and care
Qualifications
- Minimum of 6 months experience as NP/PA with geriatric or hospice/palliative exposure.
- Experience in nursing home settings is strongly preferred.
- Ability to travel statewide as directed and agreed.
- Strong communication skills with patients, families, and care teams.
- Active NP or PA license and board certification; DEA preferred.
Responsibilities
- Provide collaborative primary care to Longevity Health Plan members in nursing home settings.
- Execute Model of Care activities and advanced care planning.
- Coordinate with care teams; document plans and changes clearly.
- Engage families and PCPs to align goals of care and needs.
- Balance caseloads and support market goals with team members.
