Job summary

Longevity Health seeks an Advanced Practice Practitioner (NP/PA) to serve as a Market Floater for Longevity Health Plan members. The role requires licensure, board certification, and the ability to travel statewide to support care in multiple facilities, with duties spanning primary care, care planning, and coordination with PCPs and care teams.

Qualifications

• Active state license as NP or PA and board certification
• Minimum of 6 months experience as NP/PA with geriatric/hospice/palliative experience preferred
• Willingness to travel statewide
• DEA license and active NPI and Medicare provider number required
• Excellent clinical skills in assessment, diagnosis, and treatment, with the chronically ill population in mind

Responsibilities

• Provides collaborative primary care to Longevity Health Plan members
• Delivers Model of Care activities and care planning
• Facilitates operations: prior authorization, Part A/B services, medication reviews
• Communicates with members, families, PCPs, and care teams
• Supports market goals and balances caseloads with team needs
• Promotes Longevity Health Plan to stakeholders
• Works autonomously and maintains accountability in nursing home environments
