Job summary

Longevity Health seeks an Advanced Practice Practitioner Market Floater (NP/PA) to provide primary care in nursing facilities across markets. You will develop individualized care plans, participate in care team meetings, and coordinate with families and PCPs to meet each resident's goals.

This role requires travel and autonomous practice within a supportive clinical structure. Responsibilities include delivering the Longevity Health Plan Model of Care, documenting health status, and ensuring

Qualifications

• Minimum 6 months as NP/PA with geriatric/hospice exposure; new grads possible via fellowship.
• Active state license and board certification; DEA license required.
• Ability to travel statewide and work autonomously within a team.

Responsibilities

• Provide collaborative primary care to a panel of Longevity Health Plan members in nursing facilities.
• Deliver Model of Care activities and comprehensive care planning.
• Coordinate with PCPs, families, and care teams; participate in facility meetings.
• Document status, diagnoses, and care plans; monitor changes and adjust as needed.
• Support clinical operations: prior authorization, POS/STAR measures, and quality activities.
• Communicate with telehealth partners and family members about care needs.
• As needed, travel within assigned market to cover facilities.
