Job summary

Longevity Health is seeking an Advanced Practice Practitioner (APP) – Market Floater to manage a panel of Longevity Health Plan members across nursing facilities. The APP will perform comprehensive assessments, develop individualized plans of care, and collaborate with primary care teams to ensure high-quality, in-place care for chronically ill residents.

Applicants should be licensed NPs or PAs with geriatric or hospice/palliative experience, able to travel statewide, and capable of operating

Qualifications

• Nurse Practitioner or Physician Assistant with geriatric or palliative care experience preferred.
• Statewide travel required and ability to work autonomously.
• Strong communication and interpersonal skills with patients, families, and care teams.

Responsibilities

• Provide collaborative primary care to Longevity Health Plan members, in the most appropriate setting.
• Deliver Model of Care activities, advance care planning, and in-place treatment.
• Coordinate with care teams, PCPs, families, and nursing facilities.
• Attend nursing facility care meetings and communicate member goals of care.
