The Fund for Public Health in New York City (FPHNYC) is seeking a Gender-Affirming Care Access Hub Navigator in New York City. The role involves connecting callers to gender-affirming care and services, making referrals, and using approved scripts to assess needs and coordinate care.

A strong commitment to sexual and reproductive health rights is essential. Expect a 35-hour work week, Mon-Fri, with partial telework possible after probation.

Formación

• High school graduation or equivalent with three years of community work experience or related duties.
• Education/experience equivalent to above as an alternative.

Responsabilidades

• Answer calls and chats with NYC residents seeking information or referrals to gender-affirming care providers.
• Determine care needs based on location, schedule, and insurance status; assess for additional referrals.
• Provide warm handoffs to providers or connect with support services as needed.
• Collect and record information into the data system following scripts and protocols.
• Maintain daily communication with supervisor and obtain telephone consent for referrals.
• Protect and maintain individuals’ privacy and confidentiality.
