Job summary

Affinity Hospice Holdings, LLC is seeking a registered nurse/case manager to plan, organize, direct, and provide hospice care in home and community settings. You will lead the interdisciplinary team and coordinate with physicians and families to tailor care plans.

The role requires an active RN license, CPR certification, and strong communication, observation, and problem-solving skills. Two years of hospice or home health experience is preferred, with reliable transportation and driving

Qualifications

Graduate of an accredited Nursing program (Diploma/AS/Bachelor).
Current State RN license.
Current CPR certification.
Two years RN experience in clinical care, home health or hospice preferred.

Responsibilities

Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
Implement/develop/document the patient plan of care.
Provide care while following infection control measures to protect staff and patient.
Ensure continuity of quality patient care with proper documentation.
Monitor cases for third-party payer requirements.
Supervise hospice aides and licensed practical nurses.
Perform on-call duties as assigned.
