Job summary

AWARE is seeking a Care Navigator in Helena, MT, full-time, starting at $17.76 per hour. The role focuses on connecting clients with community resources, scheduling, and addressing barriers to care, with emphasis on compassionate, culturally aware service.

You will contact clients by phone, assist with appointments, coordinate with Service Teams, and maintain accurate documentation in the electronic medical record.

Qualifications

• High School Diploma or equivalent required.
• Experience supporting families and delivering client-centered services.
• Ability to document interventions in EMR accurately and timely.

Responsibilities

• Understand healthy family dynamics and child development to support families.
• Relate positively to families needing support services.
• Call clients to set up appointments and send reminders.
• Help clients overcome barriers to attend appointments.
• Address insurance concerns by connecting with resources.
• Document interventions in the EMR.
• Develop strong relationships with Service Teams.
• Assist in coordinating appointments and activities across service lines.
• Build relationships with clients, families, and colleagues.
