AWARE Inc. is seeking a Care Navigator to help individuals and families access services and supports within the community.

The role focuses on reducing barriers to care, coordinating appointments, and connecting clients with resources to promote growth and well-being. You will work within a compassionate, inclusive team, maintaining strong relationships with clients, families, and care partners while documenting interactions in the medical record and contributing to the organization’s culture of

Qualifications

• Must show sensitivity to cultural differences among clients and staff.
• Ability to work in partnership with teammates in a joint approach.
• Strong verbal and written communication skills are required.
• Ability to manage time, set priorities, organize, and follow routines.
• Commitment to uphold ethical and professional Social Work standards.
• Excellent interpersonal skills and capacity to build relationships with diverse populations.
• Experience or capability to engage with government officials, healthcare providers, and organizations.

Responsibilities

• Contact clients by phone to assist in setting appointments or send reminders.
• Provide navigation for clients, addressing barriers like logistics and childcare.
• Address insurance concerns by connecting clients with resources and team members.
• Document every intervention in the electronic medical record.
• Develop strong working relationships with service teams and coordinate with other lines.
• Answer phones, greet visitors, maintain filing, and assist in coordinating appointments.
