Qualifications

Responsibilities

• Coordinate meetings with case managers, clinicians, and providers.
• Maintain caseload and monitor service progress according to plan.
• Assess and determine need for case management services.
• Develop and monitor Individual Treatment Plans (ITP).
• Ensure enrollment in SDMI program where required.
