AWARE Inc. is seeking a Care Navigator to support adults, children and families in the Missoula community. You will help clients access services, schedule appointments and resolve barriers to care, while maintaining compassionate, client-centered service.

Qualifications

• Bachelor’s degree in business, social work, human services, or related field, or eight years of related employment experience is preferred.

Responsibilities

• Understand healthy family dynamics, conflict resolution, problem solving, and child development; maintain professional office practices.
• Relate positively to families needing support services and establish nurturing relationships.
• Contact clients by phone to set up appointments or send reminders.
• Provide navigation for clients by addressing logistic barriers, scheduling issues, childcare needs, etc.
• Address insurance concerns by connecting patients with resources and team members.
• Document interventions in the electronic medical record.
• Develop and maintain relationships with Service Teams and other organizations.
• Answer phones, receive visitors, maintain filing systems, assist in coordinating appointments and activities.
