Job summary

Responsibilities

• Performs case management and concurrent planning in Foster Care per policy.
• Maintains case records and documents per DSS policy.
• Engages families and collaborates with professionals and legal teams.
• Provides on-call coverage as assigned and follows GPS Practice Model.
• Ensures safety, permanency, and well-being through team-based planning.
