Zusammenfassung

The South Carolina Department of Social Services is recruiting a CWS Case Manager TSS to support current and former foster young adults in the Extension of Foster Care program. You will provide case management, service navigation, and ongoing transitions planning as part of the Transition Services and Support Division.

You will engage with young people, document activities in CAPSS, collaborate with legal for compliance, and participate in on-call duties, with a strong focus on safety, stability

Qualifikationen

• Master's degree and 1 year of professional experience in social services or related field.
• Bachelor's degree and 3 years of professional experience in social services or related field.
• Prefer child welfare experience.
• Must have a valid driver's license and be willing to drive and transport children and adults.

Aufgaben

• Perform case management for foster young adults to ensure growth, stability and safety.
• Maintain monthly face-to-face and frequent virtual contact with participants.
• Document all activities in CAPSS per policy and standards.
• Provide service navigation and timely referrals to community resources.
• Engage families and build connections to support systems.
• Prepare participants for Young Adult Transitional Teaming and court proceedings.
