Job summary

Mental Health Resource Center, Inc. is a not-for-profit Florida corporation delivering mental health and behavioral health services, including 24-hour emergency care and outpatient programs.

The FACT Case Manager role supports stabilization and community living, working within a multidisciplinary team to provide advocacy, case management, and recovery-focused care. Most services occur in the community with some office time; team members use company vehicles and may be reimbursed for mileage.

Qualifications

• Bachelor’s degree in Human Services or a related field required.

Responsibilities

• Provides treatment, rehabilitation, and support services in accordance with the FACT model (based on the PACT model).
• Maintains accurate, complete, and confidential clinical records.
• Conducts mental health status assessments.
• Coordinates referrals to community providers and support services.
• Assists individuals with activities of daily living, transportation, communication, and social/interpersonal skills.
• Provides education on mental health symptoms, medications, and recovery.
• Serves as primary case manager for an assigned caseload, promoting recovery, empowerment, and self-determination.
