Mental Health Resource Center, a not-for-profit Florida corporation, seeks a FACT Case Manager to provide treatment, rehabilitation, and supportive services to adults with severe mental illness. The role emphasizes recovery, empowerment, and active participation in treatment planning, with a multidisciplinary team and community-based service delivery.

Most work occurs in the community, with some office time; company vehicles are available and mileage is reimbursed.

Qualifications

• Bachelor's degree in Human Services or related field is required.
• Experience with adults with severe and persistent mental illness is required.
• Proficiency with RBHS/MHRC EHR and Patient Information System within 3 months.

Responsibilities

• Provides treatment, rehabilitation, and support services per FACT model.
• Maintains accurate, complete, confidential clinical records.
• Conducts mental health status assessments.
• Coordinates referrals to community providers and support services.
• Assists individuals with ADLs, transportation, communication, and social skills.
• Provides education on mental health symptoms, medications, and recovery.
• Serves as primary case manager for assigned caseload, promoting recovery and empowerment.
