Job summary

Mental Health Resource Center is seeking a FACT Case Manager to deliver treatment, rehabilitation, and support services to adults with severe mental illness through the FACT program. The role focuses on stabilization, community living, and participation in treatment planning.

The position involves coordinating care, documentation, and advocacy, with most services in the community and some office time. A valid Florida driver’s license and driving record are required, with mileage reimbursement

Qualifications

• Bachelor’s degree in Human Services or related field required.
• Experience working with adults with severe and persistent mental illness preferred.
• Proficiency with RBHS/MHRC EHR and Patient Information System within 3 months of employment.
• Proficiency in Microsoft Office, Outlook and Internet use.
• Must meet Florida driver requirements and have valid driver’s license.

Responsibilities

• Provide treatment, rehabilitation, and support services per the FACT model.
• Maintain accurate, confidential clinical records.
• Conduct mental health status assessments.
• Coordinate referrals to community providers and supports.
• Assist individuals with activities of daily living, transportation, communication, and social skills.
• Educate on mental health symptoms, medications, and recovery.
• Serve as primary case manager for assigned caseload to promote recovery.
