CN Guidance and Counseling Services, Inc. seeks a qualified professional to deliver services to individuals diagnosed with severe mental illness. The role emphasizes integrated mental health and substance abuse treatment within the ACT team framework.

Responsibilities include developing service plans, coordinating with jail/prison reentry, and liaising with probation officers to ensure compliance. Strong advocacy, case management, and crisis intervention skills are essential.

Qualifications

• Master's degree in Counseling or related field with LMSW/LMHC-LP/LMFT-LP/LCAT-LP credentials.
• At least 1 year of training or experience in integrated mental health and substance abuse services.
• Experience serving justice-involved or incarcerated clients is a plus.

Responsibilities

• Lead responsibility for justice-involved clients within dual-recovery and related services.
• Prepare required AOT, parole, probation, or court progress reports and accompany clients to hearings.
• Engage family, housing providers, defense attorneys, and community corrections to ensure regulatory compliance.
• Develop service plan goals to reduce criminogenic risk and promote prosocial living.
• Coordinate reentry activities with jail/prison coordinators and visit clients during detention.
• Coordinate with probation and parole officers and other agencies as needed.
• Provide assessment, service planning, QA, utilization review, and incident reporting.
• Conduct assessments, case management, crisis intervention, and discharge planning.
• Maintain timely case records and documentation in compliance with regulations.
• Deliver direct services to recipients in-office or in community settings, as appropriate.
• Participate in on-call coverage and supervision to improve practice skills.
• Support expectations and resources after treatment through proper discharge planning.
