Mental Health Resource Center is seeking a FACT Case Manager to deliver treatment, rehabilitation, and support services to adults with severe mental illness through the FACT program. The role focuses on stabilization, community living, and participation in treatment planning.
The position involves coordinating care, documentation, and advocacy, with most services in the community and some office time. A valid Florida driver’s license and driving record are required, with mileage reimbursement
Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community. Services include 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, as well as outpatient services such as medication management, case management, and counseling.
The FACT Case Manager provides treatment, rehabilitation, and support services to individuals served by the FACT Program to support stabilization, reduce hospitalization, and promote successful community living. The Case Manager also promotes recovery, empowerment, self-determination, and active participation in treatment planning for each FACT participant.
The FACT (Forensic Assertive Community Treatment) Program is a multidisciplinary clinical team that provides comprehensive, community-based services to adults with severe and persistent mental illness. The program is designed to reduce hospitalization, increase community tenure, and enhance independence, autonomy, and quality of life.
FACT team members provide a wide range of services, including advocacy, crisis intervention, assessments, treatment planning, case management, medication support, and coordination of care. Additional services include community referrals, symptom education, vocational and educational support, social skills development, and assistance for families and caregivers.
Most services are provided in the community, with approximately 15% of time spent in the office. Company vehicles are available; however, team members must also use their personal vehicles as needed and are eligible for mileage reimbursement.
The FACT team operates during and outside of standard business hours, including rotating weekend, holiday, and on-call responsibilities.
This position is a Full Time position (rotating weekends, on-call, and holidays).
