Mental Health Resource Center, Inc. is seeking a Full Time Registered Nurse for the FACT program to conduct psychiatric assessments, coordinate referrals, and ensure proper medication self-administration in collaboration with psychiatrists and APRNs.

The role emphasizes recovery, empowerment, and client-centered decision-making. The FACT team provides comprehensive community-based services to adults with severe mental illness, with time split between community and office settings.

Qualificações

• Associates or Bachelor's degree in Nursing is required.
• Valid Florida RN license in good standing.
• Six months of medical/surgical and/or psychiatric nursing experience.
• Proficiency in the RBHS/MHRC EHR and Patient Information System within three months of employment.
• Proficiency in Microsoft Office, Outlook and Internet usage.

Responsabilidades

• Provides treatment, rehabilitation, and support services in accordance with the FACT model.
• Maintains accurate, complete, and confidential clinical records.
• Conducts mental health status assessments.
• Coordinates referrals to community providers and support services.
• Coordinates referrals to community health care providers.
• Provides education on mental health symptoms, medications, and recovery.
• Ensures proper medication self-administration in conjunction with the ARNP or Psychiatrist.
• Serves as primary case manager for an assigned caseload, promoting recovery, empowerment, and self-determination.
