Job summary

Mental Health Resource Center offers a FACT Case Manager position in Florida, supporting adults with severe mental illness through a multidisciplinary team focused on recovery, empowerment, and community integration. The role includes coordination of care, advocacy, and treatment planning.

Most duties are in community settings with some office time; benefits include mileage reimbursement and flexible scheduling to cover rotating weekends, holidays, and on-call shifts.

Qualifications

• Bachelor’s degree in Human Services or related field is required.
• Experience working with adults with severe and persistent mental illness is required.
• Proficiency in RBHS/MHRC EHR and Patient Information System demonstrated within three months of employment.
• Proficiency in Microsoft Office, Outlook and use of the Internet.
• Must meet MHRC/RBHS driver requirements, including a valid Florida driver’s license and acceptable driving record.
• Ability to work in homes, offices, hospitals, nursing homes, and secure units.
• Strong communication and interpersonal skills and ability to work with individuals, families, providers, and team members.

Responsibilities

• Provides treatment, rehabilitation, and support services in accordance with the FACT model.
• Maintains accurate, complete, and confidential clinical records.
• Conducts mental health status assessments.
• Coordinates referrals to community providers and support services.
• Assists individuals with activities of daily living, transportation, communication, and social skills.
• Provides education on mental health symptoms, medications, and recovery.
• Acts as primary case manager for an assigned caseload and promotes recovery and self-determination.
