Job summary

Mental Health Resource Center, Inc. is seeking a FACT Case Manager to deliver community-based treatment, rehabilitation, and support services for adults with severe mental illness.

The role focuses on stabilization, recovery, and active participation in treatment planning within a multidisciplinary FACT team. Responsibilities include advocacy, crisis intervention, assessments, care coordination, and education on symptoms and medications.

Qualifications

• Bachelor’s degree in Human Services or related field required.
• Experience with adults with severe and persistent mental illness required.
• Proficiency with RBHS/MHRC EHR and basic information systems within three months.

Responsibilities

• Provides treatment, rehabilitation, and support services in accordance with the FACT model (PACT-based).
• Maintains accurate, complete, and confidential clinical records.
• Conducts mental health status assessments.
• Coordinates referrals to community providers and support services.
• Assists individuals with activities of daily living, transportation, communication, and social/interpersonal skills.
• Provides education on mental health symptoms, medications, and recovery.
• Serves as primary case manager for an assigned caseload, promoting recovery and self-determination.
