Job summary

Quality Care LLC seeks a dedicated Care Coordinator / Direct Service Professional to join a hybrid team serving clients in Minnesota. You will split time between coordinating administrative tasks and delivering compassionate in-home support.

Responsibilities include documenting client services, scheduling, coordinating communications, and providing companionship and daily living assistance. A valid driver's license and reliable transportation are required, with flexibility in scheduling to meet

Qualifications

High school diploma or equivalent
Strong organizational and communication skills
Experience with office software and ability to handle paperwork accurately
Compassionate, patient, reliable and detail-oriented
Previous caregiving or direct support experience preferred but not required
Valid driver's license and reliable transportation

Responsibilities

Complete paperwork and documentation related to client services, including data entry and filing
Support scheduling, communications, and office organization to ensure smooth daily operations
Provide companionship, emotional support, and assistance with daily living activities such as meal preparation, housekeeping, and transportation
Help clients develop and maintain independent living skills in their home environment
Collaborate with other Care Coordinators and staff to promote client well-being
