Job summary

Adara Home Health is seeking an RN Case Manager to deliver skilled nursing care at client homes across Ramsey, MN and nearby communities. The role includes clinical oversight, care planning, and collaboration with families and the health care team.

You will manage 1:1 client care visits, travel between homes, and participate in on-call rotations with a focus on quality outcomes and compliant documentation.

Qualifications

• Licensed RN in Minnesota with no restrictions.
• TB test or chest x-ray as required.
• Access to reliable transportation for client visits.
• US citizenship or valid work permit.
• Background checks and screenings completed.

Responsibilities

• Provide direct nursing care and clinical oversight to home health clients.
• Develop and update home health plans of care with the care team every 60 days or as needed.
• Collaborate with physicians and home health staff to ensure quality outcomes.
• Document accurately and in a timely manner per payer and regulatory requirements.
• Educate clients, families, and caregivers on care plans and safety.
