Adara Home Health, Inc. is hiring an RN Case Manager to deliver direct patient care and oversee home health services in the Buffalo, MN area, serving Brooklyn Park, Champlin and surrounding cities. You will collaborate with families and care teams to implement goal-centered plans and ensure compliant documentation.

The role requires MN RN licensure, travel to client homes, and strong clinical supervision. Base salary ranges from $70,000 to $95,000 per year plus productivity and benefits.

Qualifications

• RN license in Minnesota without restrictions.
• Pass initial and ongoing background checks and screenings.

Responsibilities

• Provide direct patient care and oversee home health services.
• Collaborate with client, family and care team to develop a goal-centered plan.
• Ensure documentation accuracy and compliance with payer/billing requirements.
• Provide education to client, family, and staff.
• Coordinate care with physicians and home health team; supervise licensed/unlicensed staff.
