Adara Home Health is seeking a Clinical Admissions Nurse to ensure client care aligns with policies and procedures. You will maintain strong referral-source relationships and report to the Clinical Supervisor while traveling to client homes for 1:1 care.

The role requires Minnesota RN licensure, 2+ years in health care, and the ability to manage care plans, document nursing notes, and educate clients and families.

Qualifications

• Currently licensed as a Registered Nurse in Minnesota.
• At least two years of experience in a healthcare facility or home care.
• Strong organizational skills and ability to work independently.
• Demonstrates ability to make appropriate clinical judgments.
• Excellent written and oral communication skills; teaching skills.

Responsabilités

• Perform initial and periodic assessments to determine client needs and implement care.
• Record nursing diagnoses, outcomes, interventions and notes.
• Develops and updates care plans as client condition changes.
• Provide total client care including medications and treatments as ordered.
• Coordinate client care with family, other caregivers and disciplines.
• Maintain confidentiality and infection control.
• Participate in after-hours on-call processes to ensure policies and staffing.
