Job summary

Adara Home Health, Inc. is seeking an RN Case Manager to deliver direct nursing care and oversee home health services for clients in the Buffalo, MN area, including Brooklyn Park and nearby cities.

You will collaborate with families and the health care team to create goal-centered care plans, ensure quality outcomes, and manage documentation for payer billing. The role reports to the Clinical Supervisor and offers salary plus productivity bonuses and a comprehensive benefits package.

Qualifications

• Currently licensed as a RN in Minnesota in good standing.
• Proof of negative TB testing (Mantoux, TB blood test, or chest x-ray).
• Access to a dependable automobile for travel between client homes.
• Valid driver’s license and auto insurance when using personal vehicle for work.
• US Citizenship or valid Alien Work Permit evidence.

Responsibilities

• Reflect the company’s mission in professional relationships.
• Perform initial and periodic client assessments and implement care plans.
• Collaborate with physicians and the home health team to update the plan every 60 days or when changes occur.
• Provide direct nursing care to clients with varying needs.
• Coordinate care with families, caregivers, and community resources.
• Oversee licensed and unlicensed staff; delegate as needed.
