Job summary

Adara Home Health is seeking an RN Case Manager to provide direct nursing care and oversee home health services within the Bloomington area. The role collaborates with families and the health care team to develop goal-centered plans, with supervision from the Clinical Supervisor.

You will travel to client homes for 1:1 care, manage 1-3 hour visits, and participate in an on-call rotation while reporting to Mendota Heights, MN.

Qualifications

• Must be licensed as an RN in Minnesota.
• Must provide proof of negative TB testing.
• Own reliable automobile and valid driver's license.
• U.S. citizenship or valid Alien Work Permit.
• Background checks required including MDH & OIG lists.
• Minimum 2 years of healthcare facility or home care experience.
• Ability to communicate effectively and educate clients and families.

Responsibilities

• Develop and maintain professional relationships.
• Assess clients, determine care needs, implement interventions, and evaluate outcomes.
• Collaborate with physicians and home health team to update the care plan every 60 days.
• Advocate for clients' safety and report abuse or exploitation.
• Maintain accurate documentation for payer and billing compliance.
• Provide client education to clients, families and caregivers.
• Provide direct nursing care and clinical oversight to staff.
• Coordinate care with family and community resources.
• Ensure effective financial management of home health services.
• Participate in on-call rotation and manage schedule.
